    Wound care, dressing changes, observation, and reporting provided when clinically appropriate, authorized, and included in the client's plan of care.

  • Health Observation, Monitoring & Reporting

    Observation of health status, symptoms, and vital signs when appropriate, along with reporting of significant changes to the client, family, or authorized provider.

  • Catheter Care

    Catheter-related nursing care may be provided when clinically appropriate, authorized, and supported by the client's plan of care.

  • Post-Hospital & Post-Procedure Nursing Support

    Nursing support at home following a hospital stay or procedure, when appropriate to the client's needs and authorized plan of care. Services may include observation, medication support, care-plan follow-through, and recovery-related support within WestCare's scope of services.

  • Client Education & Care Coordination

    Education for clients and families, along with communication and care coordination with authorized physicians and other providers when appropriate.

  • Personal Care & ADL Support

    Assistance with activities of daily living may be provided alongside nursing services under the individualized care plan and appropriate RN oversight.
